When you tag your story with the "Lesbian romance" tag, what does that mean? Does it mean the two genetic women have a romance, or does it mean that a MTF (Transwoman) has a romance with a genetic woman?

Drop the prefixes please
One would assume two women. Why are the prefixes even mentioned?
Actually why bother with the term lesbian? A well written romance makes any gender or sexual orientation tag superfluous.
If you want an example read 'Last of the summer wine' by Mary Renault.
However political correctness can only go so far. If it describes what you want to read accurately who cares?

but a lesbian romance could include any two people who identify as women - two genetic women, one genetic and one trans, or even two trans women.
In my case
I reserve it for two women, whether trans by surgery, magic, or sci-fi, having a romance. (New Co-Ed, The Things We Do For Love, etc). Women are women once a transformation or surgery are complete. One could, I suppose, argue that a pre-op trans having a love affair with a woman is lesbian, at least in her mind, but I haven't ventured into that territory yet.

That's the trouble with labels. They mean different things to different people.

if two persons both self-identifying as women get it on, it's girl-on-girl action! ^_^

diversity counts
I'd open the spectrum wider than most. The point about "in the mind" is what counts for sure. I'd argue that two crossdressers can fall into a lesbian relationship - because it's in their minds that they are female. They don't have to be pre-op transsexuals to qualify; though two pre-op girls can, of course, have a lesbian love affair. Two post-op girls who get together will, of course, be lesbians.
Do a CD husband and his wife - if she's willing and supportive - not qualify? I could argue they do, as much as a dyke and femme lesbian couple would qualify - they're lesbians, not (unless they choose to think differently) man and woman. Why all the labels??! When I tag my writings with lesbian romance, it's because two characters see themselves as loving ladies.
