Would the people at school even know that there was a difference about Joan, Josh and her family, as their perception of reality has been changed? I thought only Megan and Mindy were able to see the real reality because they were witches? Just wondering I guess.

...a while back, I think, that in this worldline John and Josh were born as male twins, and that John has just become Joan. So that's the difference that they'll spot in school.
